GRADE 9-12 Health
Unit 4: Healthy Nutrition with Physical Activity

1.       Unit Goals (long range—how does this unit fit with year-long goals?)
  • Students will create and track a fitness goal for the unit.
  • Students will create and track a nutrition goal for the unit.
  • Students will model the importance of selecting nutritional healthy meals.
  • Students will model and participate in a physical activity from each of the physical components.
  • Students will evaluate and write a reflective paper after completing nutritional plan and fitness log.
2.       Specific ways to relate goal(s) to curriculum (team/department, system and state frameworks)
  • Students will have read unit 4: Healthy Nutrition and Physical Activity Unit
  • 9-12.H.5.1.6 Defend the healthy choice when making decisions.
  • 9-12.H.5.1.7 Evaluate the effectiveness of health-related decisions.
  • 9-12.H.6.1.1 Assess personal health practices and overall health status. 
  • 9-12.H.6.1.2 Develop a plan to attain a personal health goal that addresses strengths, needs, and risks. 
  • 9-12.H.6.1.3 Implement strategies and monitor progress in achieving a personal health goal. 
  • 9-12.H.6.1.4 Formulate an effective long-term personal health plan. 
  • 9-12.H.7.1.2 Demonstrate a variety of healthy practices and behaviors that will maintain or improve the health of self and others.

3.       Characteristics of the students for whom this unit is intended (needs in the cognitive, affective, physical, and social areas)
  • This unit is intended for students’ grades 9-12.
  • This unit is intended for students’ of all abilities.

4.       Student’s Present Level of Performance and Knowledge (Do the students have the adequate knowledge to complete the unit successfully?  What prerequisite skills must the students have to complete the unit content?  Include technology skills. 
·         Students must have basic computer skills and complete Online Orientation.
·         Students should have access to Blackboard Collaborate by enabling Java and disabling pop-up blockers.
·         Students must be in grade 9-12. 

5.       Classroom Layout and grouping of students (Where will the learning take place?  How will the room be organized?  How will students be grouped (whole group, individuals, pairs, small groups, etc.)
  • The reading material and weekly logs will be filled out online.
  • The nutrition plans will be completed and discussed with family member (preferably the one who does the meal planning/cooking/ and purchasing of food)
  • The physical activity log can be completed individually, in partners, or in groups. They will be encouraged to do at least one of their physical activities with another individual.
  • This will be a one week unit. 
6.       Introductory procedures (How will you introduce the unit and the goals?)
  • This unit takes place half way through the quarter.
  • Students will have completed units 1-3 previous to this discussing all aspects of health including mental and emotional health.
7.       Materials and media (any resource, persons or materials: be as specific as possible. Identify resource, its availability, how it is to be used and why.
·         All materials will be presented online
·         Specific instructions will be online within the reading content.
·         Blackboard collaborate online session will be utilized

8.       Visuals (although this is part of the materials and media, I want this to be a specific part of your plan).  Identify that you will be creating and use, their purpose, and design.

9.       Assessment and evaluation of learner understanding (Identify the methods the teacher will use to identify student learning levels and needs. Describe when these will be used during the unit)
  • Student will submit their created nutritional plan and fitness log each week during the unit.
  • Email confirmation of parent/mentor is required.
  • Fitness log will include labeling physical component of each activity.
  • Reflective journal from following nutritional meal plan and activity log.
10.   Relate assessment instruments to the outcomes stated in the goals.
·         Students will participate in the fitness plan they created.
·         Students will follow nutritional plan they created
·         Student will work out for at least 30 minutes per day 5 days a week.
·         Student will submit fitness log and nutritional plan weekly for grading.
